1) Direct Pay (time-of-service)

We are are a Direct Pay clinic.  What that means is that payment is made by you at the time-of-service.  This method saves us time and money and we pass the savings along to you by offering a 10-20% discount on all services.  If you plan on submitting for insurance reimbursement, we will provide you with a receipt that contains all the necessary information to submit to your insurance company for direct reimbursement.  Its easier than you think, and many private health plans like Premera, Lifewise, Aetna, Cigna, Regence, Group Health and others, offer out-of-network benefits for our services.  Many our our patients discover that they actually save money when paying directly and submitting to their insurance company on their own.  We can provide you with a worksheet that will help to clarify the benefits you have at our office.

2) Auto Accident and Work Injury Claims

For Open/Active claims only: We offer direct billing for Auto Accident  (PIP) and Work Related Injuries (Labor & Industries claims)

We will accurately submit bills with any necessary documentation to your insurance company.  All insurance payments made to The Vital Energy Center on your behalf will be applied to your account upon receipt.  Any portion of the bill not covered by your insurance company will be the patients responsibility.

If you do not have an open/active claim for personal injury protection (PIP) and/or have a 3rd Party Claim, we require payment at the time-of-service.

3) Direct Pay Fee Schedule

Chiropractic care with Jared Kohler BSc, DC, BCAO

New member 1st visit:   130 – 300  >>   60+ minutes:  Thorough consultation and evaluation.  This fee is based on the complexity of the history and condition and the number of areas evaluated.

Regular visit: 65 – 115    >>  10 – 25 minutes: Includes all spinal & extremity adjustments, manual muscle testing & neuromuscular rehabilitation, traction, exercises &  self-care education.

Re-evaluations & Coordination of care:  65 – 150    >>  10-30 minutes: Time spent for consultation & coordination of care and for progress evaluations & the assessment of new injuries and conditions.

Digital X-rays and Analysis:

  • Atlas Orthogonal Set (4 films): 180

  • Thoracic Spine : middle back (2 films): 110

  • Lumbar : lower back (2 films): 110

  • Any region: (1 film): 65

Nutritional consultations with Carolyn Story MS-Ed, NTP

  • 1-hour session =97

  • First visit special: =  57 >>  Your first one hour session with any of our amazing therapists!

  • Manual Ligament Therapy with Stephen Wadsworth LMT:  ½ hour= 50 1 hour= 95

  • Massage Therapy with Loryn Kezer LMT :  1 hour= 80

Report Writing:

  • Billed in 15 minute increments: 25 >> includes case research and file review
